The faculty and staff are looking toward the upcoming school year as it relates to athletics with safest practices and the well-being of our learners at the forefront of our concerns. We are aware of the National Federation of State High School Association’s guidance for athletics and activities and are planning for learners to return and participate in activities in the safest ways possible. As situations change and new information is communicated, we as a staff will consider and plan accordingly. Please be on the look out for announcements regarding athletics and activities as we know more.
For the 2021-2022 academic year, we will be offering a boys and girls basketball team, girls volleyball, cross country, Girls Soccer, and a cheerleading team. There are physical, academic, and behavioral requirements to be part of a team and to participate in a team.

Athletic Clearance Website
All paperwork is submitted online via the AthleticClearance.com site. Applications are only good for one year, so if you played last year, you will need to submit a new application.
When you go to the website, select Florida. You will then need to create a new account or login if you already have one. Once you have logged on, follow the steps below to submit your paperwork.
- Click on “Start Clearance Here!”
- Select “2021-2022” for the Year.
- Select “Patel (Tampa)” as the School
- Select your sport
- Click Submit and then complete Steps 1-5.
- Once you have finished, you will then select Documents Library to upload the required paperwork listed below.
POLICIES/PROCEDURES AND FORMS
Prior to participating on a team from Dr. Kiran C. Patel High School, athletes must provide the Athletic Department with the following:
- Heath insurance information
- FHSAA Pre-Participation Physical Evaluation EL-2
- FHSAA Consent and Release from Liability Certificate EL-3
- FHSAA Pre-participation Video Certificates (Heat Illness, Cardiac Arrest, Concussion)
All forms are to be filled out and uploaded to the Athletic Clearance Website; including: the pre-participation physical (form EL-2 above), Pre-participation consent (form EL-3 above), your insurance card, and the 3 certificates from videos watched on FHSAA website (#4 above). All forms and information must be complete to be eligible to play on a team.
Playing sports at another school
Some students have brought the FHSAA form EL12 to us to sign so that they can play sports at another school. However, this form is for private schools, and we’re not a private school and hence cannot sign it.
If you want your student to play sports at the school you are zoned for, the FHSAA has told us that you simply have to provide proof to your zoned school that your student is attending PHS. A student schedule can suffice, or another document that has your student’s name and something related to their enrollment at PHS.
If you want your student to play at a school you aren’t zoned for, we’re told that the EL14 form needs to be filled out, which can be found on their website here.
